Regardless of which office your appointment is scheduled for, we
ask that you be considerate with the following requests:

should have received a packet of information and forms in the
mail...please complete and fax back to us PRIOR to your
appointment. This allows us to get you set up in our new
Electronic Medical Records system and save you time when you
arrive.

Arrive
20 minutes BEFORE your scheduled time.

Please
be prepared to pay your insurance co-payment upon arrival. Should
you have any questions about our
Financial Policy, please call us.
